Wound Care Nursing
  Our Certified Wound Care Nurse provides our patients with the most up-to-date, state-of-the-art techniques and expertise for treating wound and ostomy problems in the comfort of home.

Diabetes Education & Management
  Our nurses help you achieve optimal blood sugar levels by monitoring your medication, diet and exercise, and teaching you how to live a normal life with diabetes.

  A home nursing program for people with significant social or emotional problems, chronic illness, or long term care management needs.

Home Telehealth
  Community Nurses offers our patients better access to care through the use of telehealth units in the homes of appropriate patients. Using a telehealth unit, our patient can take his/her own blood pressure, blood sugar, weight, heart sounds, and pulse ox and easily transfer the results back to his/her nurse at our main office. Telehealth allows our staff to more closely monitor patients in-between regular scheduled home visits.

CHF (Congestive Heart Failure) Program  
CHF is one of the leading diagnoses of patients admitted to the services of Community Nurses. The CHF program enables us to monitor daily vitals using our telehealth equipment, and in turn, adjust a patient’s diuretic dose according to specified weight gain. Patients are monitored closely by our nurses and their doctors to help reduce hospitalization recurrences.
