At Community Nurses, our experienced professional Registered Nurses assess your condition on a regular basis, provide skilled services in your home, keep your physician informed of your health status, and coordinate any additional services you may need.

Oncology Nursing
Cancer patients have the security of knowing that our specially-trained nurses provide skilled services right in the comfort of their own homes, including in-home chemotherapy treatments. Our team of experienced Community Nurses professionals offers a comprehensive cancer care program for our patients.
Pediatric Nursing
Most people think of home health care as being only for the elderly, but at Community Nurses many of our patients are children. Our special pediatrics nursing program is directed at the needs of children from infancy through adolescence. Children with special physical needs can receive skilled nursing care and therapy services in the security of their home environment.
Maternal/Child Healthcare is an important part of our pediatric nursing program, offering skilled services during a woman’s pregnancy, and professional homecare for both mother and baby after early discharge from the hospital following delivery.
Telehealth
is a term that describes the
transmission of medical data and information electronically
through standard telephone lines. Community Nurses now has
over 100 telehealth units to place in the homes of our
patients. The units help monitor our patients more closely by
transmitting data on blood pressure, blood sugar levels,
weight, pulse oximetry, etc. back to the nurses at our office
on a daily basis. This allows for daily assessment and early
intervention. Patients find the units easy to use and our
medical staff is seeing improved outcomes for our patients.

Home Health
Aides |

Community Nurses’ Home Health Aides assist patients with bathing, dressing and grooming, and help your family meet your personal care needs. Home Health Aides work under the supervision of a Registered Nurse.

Medical
Social Services - including Mental Health Counseling |

There are many questions and concerns that arise with you and your family that a medical social worker can help solve. Not only does our Community Nurses’ medical social worker help to solve problems that your family might face in caring for you, but they are skilled at assessing you and your family’s ability to cope with the illness or disability. |
Nutrition

Our Community Nurses’ Nutrition Counselor is an integral part of our patients’ total plan of care. Our Nutrition Counselor assists and advises Community Nurses Staff on nutrition and therapeutic diets for our patients, and counsels patients and families on how to incorporate therapeutic diets into their lifestyle. |

Physical Therapists
Whether you have had a stroke, a hip replacement, or another injury or ailment, Community Nurses’
physical therapists use patient education, in-home treatments, and exercise programs tailored to your needs to return you to your highest functional level possible.
Occupational Therapists
It is often the Community Nurses’ occupational therapist who has just the right answer to help you cope with an injury or ailment. Whether it is opening a medicine bottle with one hand, putting on your shoes, or opening a cabinet, our occupational therapists help you work out the details of life that most people take for granted.
Speech Therapists
Most people think of speech therapists as helping persons regain their speech following a stroke, or helping to fine-tune speaking abilities, but they do a lot more. Speech therapists work with childhood disorders such as cleft palate and cerebral palsy, and they also help return people to speaking following the removal of vocal nodules or the larynx (voice box). In addition, speech therapists are skilled at correcting swallowing disorders. |
