Care Connections is a home nursing program for people with significant social or emotional problems, chronic illness, or long term care management needs. We provide one-to-one interventions to relieve, reduce, or manage problematic symptoms, improve function, and support recovery. Behavioral health services are obtained by:
- Cardiac
Home Cardiac Recovery Program offers quality of life, with the freedom of recovery in your home. It is a specialized, total approach to the patient recovery process. Community Nurses, Inc. uses standardized teaching protocols and written educational materials to enhance patient learning and increase compliance.
- Fall Prevention
Each year, falls occur in over a third of persons over age 65 and are a major cause of disability and death. Read more in this brochure to determine if you are at risk for falling and find out how you can help reduce your risk!
- IV Therapy
Our patients may require intravenous therapy for a variety of reasons:
antibiotic therapy, nutritional therapy, and hydration therapy. Community Nurses can provide insertion and maintenance of access for these therapies through peripheral or PICC access. We also are able to provide infusion therapy using central line/mediport access. Community Nurses provide all of these infusion services with a team of qualified IV trained nurses to educate and care for our patients. A dietician is also available to ensure each patient's nutritional/hydration needs are met.

- Pediatrics
Most people think of home health care as being only for the elderly, but at Community Nurses many of our patients are children. Our special pediatrics nursing program is directed at the needs of children from infancy through adolescence. Children with special physical needs can receive skilled nursing care and therapy services in the security of their home environment. Maternal/Child Healthcare is an important part of our pediatric nursing program, offering skilled services during a woman's pregnancy and professional homecare for both mother and baby after early discharge from the hospital following delivery.
- Wound Care
Our certified Wound Care Nurses provide our patients with the most up-to-date, state-of-the-art techniques and expertise for treating wound and ostomy problems in the comfort of your home. Wound nurses provide assessment, recommendations and patient and family education to help you heal faster.
