To begin getting home healthcare services from the Community Nurses just call 1-800-841-9397 or 814-781-1415 and speak with our intake nurse.

We will contact the patient’s physicians for any necessary authorization of services, verify insurance coverage and arrange for the first home visit.

Our staff will work with the patient, family members and other health care providers to help determine the services you need and then to develop a personalized, comprehensive care plan.
